Instructions for the form:

Enter your IAS email address and agree to Wolfram's Privacy Policy
Check the box for Faculty/Staff
Select the Mathematica products you want to access
Select Single User and select your field of study for Department
Click Submit
You will be directed to sign in to your Wolfram account to download the installers
