Form I-94 Arrival/Departure Record

The Form I-94 is the Arrival/Departure Record and is a small white card indicating the visa status of an individual and the length of time an individual may remain inside the United States. A Form I-94 is issued each time one enters the United States and should be surrendered each time one departs the United States. If your Form I-94 was not removed when you departed the United States, it is possible that your departure was not recorded properly.  In order to avoid admission difficulties on future travels to the United States, please send an explanation letter, your Form I-94, and any documentation that proves you left the United States to:

It is recommended you keep a copy of what you send to DHS-CBP and carry it with you the next time you travel to the United States in case the CBP officer has any questions about your eligibility to enter. Carrying those materials with you will also allow your record to be corrected at the time of entry if the London Kentucky office has not yet done so.
